How do I Obtain My Certificate of Identity From the DOJ?
- 1). Obtain Form DOJ 361, the "Certificate of Identity Form" by contacting the Department of Justice or downloading the form from the Internet (see the resources section). Note that some agencies, such as the U.S. Citizenship and Immigration Services (USCIS) will send you this form if the FOIA or PA request does not contain a Certification of Identity.
- 2). Fill out the form by writing in the requested information on the lines provided. You need to provide your full name, citizenship, Social Security number, current address, date and place of birth. Sign the form.
- 3). Send the form back to the agency requesting a Certification of Identity. The agency will send you an acknowledgment letter indicating that it has received your Certification of Identity.
