How to Correct a Birth Certificate
- 1). Provide proof of your identity. Most states only allow you to make correction on your own birth certificate or that of your biological or adopted child. Before you may submit your request for corrections, you will need to show photo ID and prove your relationship to the person listed on the birth certificate.
- 2). Get documentation that backs up your correction. You may be asked to provide medical records, identifying documents for subject's parents, official paternity test results, or other written proof that the information listed on the birth certificate is incorrect.
- 3). If known, state the reason that the information appeared incorrectly on the original birth certificate and certify that you are not asking the record to be modified for fraudulent reasons. You will need to sign and date your request form as verification that the information you list is true to the best of your knowledge.
- 4). Obtain a court order. If you are unable to obtain written documentation showing that the information listed on the birth certificate is wrong, you may need to obtain a court order in order to make the corrects. To obtain a court order, you will need to file a petition with the court and attend a hearing on the merits of your case.
