How to File an Insurance Claim for Someone Else

    • 1). Obtain a copy of the claimant's insurance policy or card. You will need the policy number, address for claims and any deductible or copay information available.

    • 2). Gather the claimant's medical bills and any payment receipts. If a provider isn't direct-billing the insurance company, it probably means the person you are helping had to pay out-of-pocket. You'll need proof of payment, which can include canceled checks, bank statements, credit card statements and paid invoices.

    • 3). Call the insurer to request claim forms. Some insurance companies will email these, others will mail them. Use the insurer's forms to avoid hassles and complications.

    • 4). Complete claim forms using all available information. Usually, forms require a policy number, Social Security number, address, contact number, dates and amounts of services, and contact information. Include any power-of-attorney or other documents that allow you to discuss matters with the insurance company. If you do not have these, you can still file the claim, but the insurance company won't be able to discuss any issues with you when the claim is processed.

    • 5). Enclose copies of all bills and proofs of payment before mailing to the insurance company's claims processing department. If the claims address is not on the insurance card or policy you have, call the company's customer service line to get the correct address. Use mail or a courier that can track delivery to ensure your package is delivered and to provide you proof of filing.
