    • 1). Verify that you are eligible for COBRA coverage. Determine if your reason for leaving the company falls under eligible qualifying events such as voluntary resignation or involuntary ending of employment for any reason other than gross misconduct. Contact your employer if you are unsure of your qualifying event.

    • 2). Wait 14 days from the date of termination to receive the COBRA application in the mail. Call your employer to request the COBRA paperwork if you do not receive it within 14 days.

    • 3). Complete the application. Fill out the basic information such as your name, dependents and employer information. Choose the types of insurance coverage you want to continue such as health, vision and dental. Mail the completed application to the insurance company.
