• 1). Log on to the Florida Department of Health website (see References) and find Form DH261. This is the application for marriage record.
  
  
• 2). Complete the application. You can do so online and print the form.
  
  
• 3). Sign the completed application and mail it to the address listed on the form along with applicable fees. Fees should be in the form of a check or money order payable to the Bureau of Vital Statistics. If you are unsure of the date of the wedding, add an extra $2 fee for each additional year you want to search. Allow two to three weeks for delivery. Rush delivery is available for an additional $10 fee.

• 1). Call VitalChek at (877) 550-7330 to place an order by phone.
  
  
• 2). Provide the customer service representative with information about the marriage record being requested. This information includes the full names of the husband and wife, the date of the marriage and the city or county where the marriage license was issued.
  
  
• 3). Provide information about the person requesting the marriage record. Such information includes the full name, mailing address and telephone number.
  
  
• 4). Pay applicable fees with a credit card. As of March 2010, there was a $10 rush fee, a $7 service fee and a $5 marriage license fee.
