How to Get Copy of a Birth Certificate From Minnesota

    • 1). Print out the "Birth Certificate Application Form" located on the Minnesota Department of Health website, linked in the Resources section.

    • 2). Fill out parts I, II and III of the application form. Part I will ask for information on the person the birth certificate request is for. Be sure to fully fill out this section with accurate information. On part II of the application, you will need to select your relationship to the person you are requesting a birth certificate for. If you are requesting a birth certificate for yourself, check the "I am the subject" option. Part III is the "Fee and Payment" section. You will need to select your services and payment type.

    • 3). Locate a notary in your area. Section IV of the application requires that the requester fill out the form in front of a notary. You will also need to have the notary sign this document. Getting a document will generally require a fee. You will need to inquiry about this fee with your local notary service.

    • 4). Fill out either a check or money order payable to the Minnesota Department of Health. You will need to fill out the check according to the amount indicated on section III of your birth certificate request form. If you are paying by credit card, fill out your credit card information on section III of the form instead.

    • 5). Mail the completed form and payment to the following address:
