- 1). Fill out the Form I-90 online at uscis.gov. You'll need to log in to the E-filing system using your user ID and password. If you don't have a user ID and password, you can create them by clicking on the "I am a new user" option on the log-in page. After completing the form, you'll get a list of any additional documentation that you may be required to submit. Pay the required fee online by credit card or deduction from your checking or savings account. The fee at the time of this writing (May 2011) is $450, which includes the biometric fee.
- 2). Submit copies of any required additional documents, such as your marriage certificate, photo ID and birth certificate. Your application will not be processed until all necessary forms have been completed, so mail them promptly after completing the online form. Mail the documents to:

You'll receive a notice of your appointment time after your application has been processed. Bring the necessary documents with you to your appointment. You'll need to bring your biometric appointment notice and the original court order granting the name change or a certified copy of the court order granting the name change or a certified copy of your marriage certificate. You'll also need to provide a valid photo ID. Only your passport, national photo identification issued by your country of citizenship, driver's license, military photo ID or state-issued photo ID will be accepted.
