Upcoming uncertainty in America as doctor numbers reach precarious levels

America is facing a substantial shortage of doctors in the long term, maybe falling short by up to 150,000 in the next twenty years. Even though the number of doctors in the US stood in excess of 950,000 towards the end of last year, changes to government health-care law through changes in the insurance industry have seen an enormous increase in the number of people entitled to medical care, and hospitals and schools are anxiously attempting to train more and more doctors. All of this is in spite of there being many residency slots that are unfilled, although fortunately, new laws will assist to remove this wastefulness; money that is remaining will be combined to permit some residencies to be filled as opposed to every vacant role being lost. The greatest problem facing those attempting to resolve the shortage of primary care doctors though, is surely the salary available to them. Michael Brennan points out that "until primary care physicians can earn 70-80% of what most specialists make without killing themselves, there will be no incentive for the best and the brightest to go into primary care".

This is the key to the lack of primary carers in the US; there is little to no financial motivation for them to work in primary care. With huge amounts of debt having graduated medical school, specialist areas, due to their promise of superior payment, are significantly more appealing than a career struggling through in primary care. This, in addition to the dearth of availability in residency jobs, and an additional lack of funding for these, mean that the United States may be required to have a drastic rethink of its healthcare arrangement in the next decade.
