How to Obtain a Medicaid Number
- 1). Obtain your Social Security Number if you are unsure of it. You will need this, along with other identifying information such as your full name and date of birth, to obtain your Medicaid number.
- 2). Locate the contact information for your local or state Medicaid office, if you are unsure of this information. The National Association of State Medicaid Directors maintains a listing of every state's Medicaid program contact information (see Resources).
- 3). Contact your local or state Medicaid office and relay the information from step 1. You will be able to obtain your Medicaid identification number by providing this information. If you need a new card, the operator can provide you with instructions for obtaining a new card.
