How to Obtain Your Service Records

    • 1). Obtain Form 180 by calling the National Archives and Records Administration at 1-866-272-6272; or write to them at 8601 Adelphi Road, College Park, MD 20740-6001. You can also go online and download the form at www.archives.gov/veterans/military-service-records/standard-form-180.html.

    • 2). Complete Standard Form 180. You will need the service person's Social Security number, date and place of birth, name at the time she served, branch of service, component (active, reserve, National Guard), approximate dates of service, rank (officer or enlisted) and whether she is deceased or retired from the military. The same information is needed if you apply online.

    • 3). Mail the form to the address that corresponds to the type of records requested. (See references.) If you apply online, you are finished once you complete the questionnaire. You can also fax the request to 314-801-9195.

    • 4). Contact the personnel center or headquarters of the appropriate branch of service if you are looking for records of veterans who were discharged, retired or died in service since 1995 (Navy), 1999 (Marine Corps) or 2005 (Air Force).

    • 5). If you want active-duty health records, call 1-800-827-1000 for the current location of the records and how to obtain them.

    • 6). If you are currently in the National Guard, contact the Adjutant General's office of the appropriate state. (See references.)
