How to Fill Out a Juvenile Contact Report
- 1). Obtain a juvenile contact report form from your police department office. The police department you work for might have this form available to you online.
- 2). Enter the minor's name, address, date of birth, race, sex, country of birth, phone number, known occupation, employer and/or school name. You'll also include his physical appearance, any tattoos or piercings, Social Security Number, parent or guardian information and whether or not the minor was under the influence of alcohol in the designated spaces on the report.
- 3). Note any drugs that the minor was using, selling or in possession of at the time of the crime, the quantity and the suspected intent for the drugs, if applicable.
- 4). Write down any additional information pertaining to the crime in the available section. Sign and date the report and file the report with your boss.
