• 1). Download and print the mail-in application from Alabama's Center for Health Statistics vital records webpage.
  
  
• 2). Fill in the application with as much information as possible to help the vital records department locate the divorce certificate.
  
  
• 3). Send the application and fee to:

• 3). Take completed application and a check or money order for $15 made payable to "Vital Records" to any county health department. A link to a map of county health departments is available through the vital records webpage.

• 1). Go to the VitalChek website (see Resources). Select "Divorce Record," then "Continue."
  
  
• 2). Provide the information requested on the website including your name and date of divorce.
  
  
• 3). Select divorce certificate. Documents ordered via VitalChek can take anywhere from three to 14 business days to arrive. An additional fee is required for VitalChek.
