• For clients receiving State Disability Assistance (SDA) or for people who have low or no income.
• Some clients may have a spend-down.
• Cash assets limited to $3000.

Call your case manager or social worker for a complete list of covered and excluded services.

Payment Intervals:

Vouchers are provided for doctor and or pharmacy (good for 30 days)

Duration of Coverage:
