How to Get Copy of a Birth Certificate in Tulsa, Oklahoma
- 1). Download an application from the Oklahoma State Department of Health website (see Resources). If you prefer, you can obtain an application in person at the Tulsa Health Department at 315 South Utica in Tulsa.
- 2). Fill out the application completely. Provide your full name at birth, your date and place of birth, your father’s full name, your mother’s full name including her maiden name and your telephone number. Sign the application.
- 3). Present a valid photo identification when you request your birth certificate. If you request your birth certificate by mail, send a legible photocopy of your identification.
- 4). Pay the fee of $15 (the amount as of June 2010). Make checks or money orders payable to the order of Vital Records Service.
- 5). Request your birth certificate in person at the Tulsa Health Department or mail your request to Vital Records Service, Oklahoma State Department of Health, 1000 Northeast 10th, Oklahoma City, OK 73117. Enclose a self-addressed, stamped envelope if you request your birth certificate by mail for use in returning it to you.
