    Social Security

    • Social Security is the first place to start because it needs your correct last name and ID number to issue retirement benefits and tax withholding. You can also use your new Social Security card to help you change your name with other institutions. File Form SS-5 with your Social Security office to put the name change in motion.

    Department of Motor Vehicles

    • Once you've updated Social Security, it's time to get a new driver's license. At the same time you get a new license, you may also be able to update your voter registration records, as well as vehicle registration and title. Check with your state department of motor vehicles to find if it requires either a certified copy or photocopy of your marriage license.

    Work Documents

    • Your workplace will need a record of your new last name so paychecks and benefits reach the right person. You may also need to review your tax withholding and make any necessary changes to reflect your married status. Check your insurance coverage and beneficiaries. Add your spouse if applicable.

    Financial Institutions

    • Visit your bank in person to change the name on your account. The bank will likely request a copy of your marriage license (a photocopy of the original should be sufficient). Contact any other companies you have credit cards with and let them know they should update their files.

    Other

    • If you have plans to travel outside the country soon, request a new passport with your name change. The rest of the places you can notify are much less stringent about requirements, but nevertheless taking care of them quickly can ease the rest of your transition. Call or send letters to your landlord or mortgage company, other insurance accounts, doctors' offices, professional licensing boards, utilities and the post office.
