9
    • 1). Before filing your complaint, first outline all of the facts of the unresolved issue with your auto insurance company. Get a copy of your policy and review your coverage to make sure that you are not in error. Have all this information ready along with your auto insurance company's Ohio address and phone number, your agent's name and phone number, and any other correspondence records you have with your auto insurance company.

    • 2). Call the Consumer Services Division of the Ohio Department of Insurance at 1-800-868-1526. Ask to speak to an automobile insurance analyst.

    • 3). The insurance analyst will ask you questions regarding your claims. They cannot provide you with legal advice, but generally will tell you if your insurance company does not appear to be doing anything wrong. If there are some suspicious facts involving your claims the analyst will send you a formal complaint form.

Alternatively you can access the form yourself online and file it electronically. Go to (hyper link also provided below under resources):

    • 4). Once your auto insurance complaint is filed, the Department of Insurance will provide a copy to your insurance company. They will be asked to resolve the issue or asked the facts as they see them. By law, your auto insurance company is required to respond to the Ohio Department of Insurance.

Once your complaint is filed it is added to the record of complaints kept by the Ohio Department of Insurance. The number of complaints each company selling auto insurance in Ohio is tracked versus the dollar amount of policies sold in Ohio.
