How to Fix Mistakes on State Taxes
- 1). Contact the state's Department of Revenue, or other state department, in person, by phone, or by logging on to its website to obtain the appropriate tax amendment forms.
- 2). Enter personal information such as your name, social security number, mailing address and phone number in the corresponding fields.
- 3). Specify the reason for submitting the amendment, such as a federal audit or net operating loss.
- 4). Enter the past tax information. Information such as the total federal and state taxable income, credits and deductions will need to be entered just as they were on the tax forms that were originally submitted.
- 5). Amend the state tax form by entering the new data in the corresponding fields. Enter the new federal and state taxable income amounts, credits and deductions.
- 6). Sign and date the form and submit it along with any payment due. Some states will allow you to pay electronically, or you can submit a check for the total due with the amended tax form.
